Position Purpose
The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation.
Position Details
Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. NO ON CALL REQUIREMENTS!
Coverage Region: Enfield
Bonus: $10,000 sign-on bonus paid out over two years.
This position is 100% home care providing services in in patients private homes.

What You Will Do
- Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation
- Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy
- Provides statistical/budgetary data to appropriate Palliative care program.
- Works collaboratively with all departments to ensure execution of quality delivery of services
- Ensures that stakeholders are satisfied with services provided; improvement plan developed when necessary.
- Develops tracking outcomes of enrolled patients and ensures information is available for review
- A commitment to quality palliative care provision and transition to hospice when appropriate
- Exercises sound clinical judgment in oversight of Palliative Care Program
- Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors,
- practices, and decisions.
- Graduate of an approved nursing education program.
- Current Registered Nurse licensure in the state of practice.
- CPR certification
- Hospice/Palliative Care certification preferred.
- Home care experience, strong clinical knowledge base, Hospice experience
- Computer skills, strong interpersonal and organizational skills.
- Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills.
- Must be able to work independently and on a team,
- Ability to perform diversified duties with time limitations,
- Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care.
- Valid driver’s license and reliable transportation required
